The eCura system is a clinical scoring tool designed to stratify curability after endoscopic submucosal dissection (ESD) for early gastric cancer. It calculates lymph node metastasis (LNM) risk based on five weighted factors: lymphatic invasion, tumor size >30 mm, positive vertical margin, venous invasion, and submucosal invasion ≥500μm.
How the Scoring Works
Each risk factor carries a point value: three points for lymphatic invasion, and one point each for the other four criteria. The total score places patients into three risk groups: low (0–1 point, 2.5% LNM risk), intermediate (2–4 points, 6.7%), and high (5–7 points, 22.7%). These estimates are based on the 2017 study published in the American Journal of Gastroenterology by Hatta et al.
