Notice of Alleged Copyright Infringement

5. A statement that the complaining party holds a good faith belief that the disputed use of the material is not permitted by the copyright owner, its agent, or the law.
6. A statement that the information in the notification is accurate and, made under penalty of perjury, that the complaining party is authorized to act for the owner of an exclusive right that is claimed to be infringed. Be aware that under Section 512(f), anyone who knowingly and materially misrepresents that material or activity is infringing may be subject to liability for damages.
